The doctor’s findings, your diagnosis, reason for visit or reason the doctor has you returning to the office will determine which of your insurance plans we are able to submit your claim. You are responsible for any unmet deductible, copayments or non-covered items and will be billed once we have received your insurance company’s explanation of benefits for any unpaid balance. If your insurance provider is not listed here, we will be happy to submit a claim on your behalf, for reimbursement to you as we require payment when services are rendered.
